Employee ID Card Request

This form is to be used to request Employee ID cards for your facility.  Each time you hire a new employee, return to this form to request a new ID card.

Please allow 10-14 days for new facility cards, and 3-4 days for additional employee cards.

This benefit is provided at no cost to active ASP Program Participants.
Qualification: You must be active on the program for 30 days.

ID Card Design.

 If you wish to design your own ID Badge, the format must be in PC format designed in either Microsoft Publisher, or Adobe Photoshop.  Size - 54 x 85.6mm.  Email your completed file to humane2@apapets.org  Please be sure to include your facility name in the subject line.

If you would like us to design your badge, please make your choices below. We will send you a proof of the design for approval before printing.

Employee Photos: If you are using photos, you must have good quality digital images of your employees.  If you do not have access to a digital camera, we recommend using 35mm film and having the photos scanned at a local copy store.

Email photos to humane2@apapets.org  You must name each file the employees name or employee number.  Example: SusanAnderson.gif or ID#23212.gif   Please send .gif, .jpg or .psd format.

Do you want an access level listed on the ID Card?
If so, we recommend "Level 1", "Level 2" or "Area 1", "Area 2" etc.

Do you want to include an employee number?
We do not recommend using Social Security numbers.

Is there a short statement or tag line you want to use on the card?

Example: "Serving Monroe County Since 1972"
